What is the Family Transitions After School Day Treatment Program?
Family Transitions, Inc. is offering an after school day treatment program. This program is designed to help children with emotional and behavioral challenges to develop and maintain positive attitudes and age appropriate behavior. Each participant receives a customized service plan to meet his or her individual needs by combining psychotherapeutic interventions, educational activities and mental health treatment. The program combines a highly structured behavioral curriculum with recreation activities that fosters self-esteem, teaches positive decision-making skills and nurtures family support and cooperation. In addition each participant receives group and individual therapy as well as recreational and educational therapy. The overall goal of this program includes, but is not limited to, retaining youth in the community by empowering the family. Through understanding, we embrace differences and encourage success.
What is the program features for Family Transitions After School Day Treatment Program?
This program is designed to educate, empower and uplift youth in the program through activities that stimulate creativity, improve self-esteem and promote positive age-appropriate behavior. Transportation is provided and is free. Our after school day program is one individual component of the year round intervention offered through our therapeutic day treatment program.

What are the Family Transitions After School Day Treatment Program eligibility criteria?
There is an eligibility criteria for this program.
- Children must be between the ages of 5-13.
- A minimum IQ of 60 is required.
- Referred youth must exhibit specific difficulties and maintain certain requirements on a continuing intermittent basis.

Services are available through Medicaid, Department of Juvenile Justice (294 Funding), private donors, self-pay, and/or the Family Assessment and Planning Team (FAPT) funding approval process.

Referrals for the Family Transitions After School Day Treatment Program may be made by family members, school personnel, social workers, psychologists and psychiatrists by calling Intake at 262-7960.
