Family Transitions, Inc. is a behavioral health organization providing a wide range of mental health services in the Central Virginia area. Founded in 2003, Family Transitions, Inc began as a small agency providing only intensive in-home services and has now grown into an organization that prides itself on serving hundreds of individuals a year.
Family Transitions, Inc. extensive expertise is applied to the challenge of ensuring high quality and cost effective services. We strive to provide individuals and their families with the services and resources needed to live healthy, fulfilled lives within their families and communities. Our structured community-based programs enhance the lives of both our clients and the community.
Family Transitions, Inc. plays an active role by participating in civic, charitable and social organizations. We value the communities that we are involved with, and seek to utilize the local workforce for employment opportunities.
Intensive In-Home Services: A family-focused program with counseling provided in the client’s home environment. This program is for children and adolescents from 4 to 21 with emotional and/or behavioral difficulties, returning from placement, and at risk of out-of-home placement. Using the family’s strengths, the program teaches the client and family coping skills to effectively manage stress and crisis situations.
Day Treatment Services: Comprehensive, intensive, and clinical treatment services for children, ages 5 through 18, who exhibit emotional and behavioral problems. Youth can take advantage of our therapeutic services while living at home and remaining involved with their local school. Counselors help clients accomplish their goals through psychotherapeutic interventions, mental health treatment, and educational activities.
We Offer Three Day Treatment Programs:
School-Based Day Treatment Services: in the school environment during school hours.
After-School Day Treatment Services: at our agency’s location from 2pm-7pm; free transportation and snacks are supplied.
Summer Day Treatment Services: at our agency’s location during the summer months; free transportation and FDA-approved meals are supplied.
Adult Mental Health Support Services: Individualized counseling and life skills training in the home and community environments to adults 18 or older who have trouble maintaining community stability and independence due to mental illness. Areas of focus include daily living skills training, accessing community resources, health and safety, financial management and social supports.
Adult Psychosocial Rehabilitation Services: A structured day support program that provides services to adults with serious mental illness. It assists those persons who needs to establish a daily routine, who need to develop a broader social network, or who need to develop life or prevocational skills in order to live independently or to maintain employment.
Funding Services are available through Medicaid, Department of Juvenile Justice (294 Funding), private donors, self-pay, and/or the Family Assessment and Planning Team (FAPT) funding approval process.
